ABOUT THE HANS FOUNDATION
 
The Hans Foundation (THF), established in 2009, is a Public Charitable Trust that works towards creating an equitable society to enhance quality of life for all through the empowerment of marginalized and underprivileged communities in India. The Hans Foundation works for the Health and Wellbeing of remote and under-served groups, with an additional focus on children, persons with disabilities and women. THF also works for holistic Education and Livelihood development of our communities. Our Two-Fold Strategy involves identifying and working within Aspirational Districts and high poverty index in the North, North-East and East regions within specific states, as well as catering to lower-income and migratory populations in cities, through urban programs.
 
Over its 15 years of existence, THF has reached more than 35 million beneficiaries through its programs. THF works with communities through direct implementation of projects on the ground in addition to providing local management and monitoring support to not-for-profit organizations in India funded through THF USA and RIST. 
 
PROJECT
 
The Hans Foundation has been operating more than 50 PHCs/MHC on self-Implementation mode since October. 2021. PHCs intervention is an innovative model of healthcare delivery that could help alleviate health disparities in vulnerable populations and individuals with chronic diseases. Indeed, some studies have concluded that PHCs/MHC are particularly impact in the following contexts: offering urgent care, providing. preventative and primary health care and initiating chronic disease management. By opening their doors directly into communities and leveraging existing community assets, PHCs/MHC can offer tailored, high impact and affordable health care that responds dynamically to the community’s evolving needs. Goal of this project is “People in remote areas can access quality health services, thereby improving their overall well being”.
 
GENERAL

KEY ACCOUNTABILITIES
 
 Project Management:
  • Setting up of districts office and responsible for its logistics procurement.
  • Ensures the regular supplies for MHC through management of supply chains.
  • Forecast the requirements of MHC to ensure timely supply of drugs, consumables etc by coordinating with concerned teams and submit the indent to regional office.
  • Ensuring high level of data maintenance for all reporting purposes.
  • Ensuring stakeholders involvement for efficient project outcomes.
  • Management and monitoring of all fixed/consumable assets
  • Tracking the MHC maintenance records like Fire/servicing/pollution/fitness etc. and provide timely information to regional office.
  • Ensures disposal of bio medical waste generate in MHC as per provision.
  • Responsible for uninterrupted operations of the MHC
  • Case study/anecdotes collection from the field with high quality photograph.
  • Ensure timely submission of mandatory reports as required.
 
 Team Management and Coordination:
  • Assist Team member in supervising the work of the MHC and other team members and is responsible for optimizing their productivity.
  • Organizing periodic review meetings with  project teams to review progress against plans, find solutions to problems, share lessons learned etc.
  • Ensures team attendance, keeping the leave records and submitting leaves and share attendance records to regional office in timely manner.
  • Plan and implement training and capacity building activities for the team members.
Monitoring, Evaluation and Reporting:
  • Oversee the data on App and ensuring that the team is updating it with accuracy.
  • Conduct periodic visit to monitor and provide supportive supervision to the team.
  • Coordinate monthly and quarterly reports and timely submission to the regional office and Health departments with the guidance of PM.
 Finance
  • Preparation for all activities/work plans for approval as per the standardize organizations financial norms.
  • Ensures timely collection, verification and submission of bills from the field to regional office like MHC maintenance, office supplies and maintenance etc. accordance with financial policies and procedures
  • Follow up for the payments for the submitted bills
  • Responsible for maintenance of petty cash book, bills, voucher etc.

Educational Qualifications
Master’s degree in social sciences/ public health or any relevant field
 
Functional / Technical Skills and Relevant Experience & Other requirements (Behavioral, Language, Certifications etc.)
